Policies for Medication and Dismissal
If a student is sick and needs to go home, the parent/guardian needs to pick up the student in the clinic. A picture identification is necessary before a student can be released to the parent/guardian. If a student is a driver, the student may be released after a phone call to the parent and a clinic card must be on file before any student may leave. If a student calls a parent from a cell phone or any other office phone, please make sure the student is instructed to go to the clinic and the student will be evaluated and then the coordination for pick up can be made from the clinic and the attendance office.
If a student needs to take medication in the clinic, an "Administration of Medication Request" form must be filled out by the parent.
If a student has asthma and needs to carry an inhaler, an "Asthma Action Plan" (by state law) must be completed and signed by the doctor stating that the student has been instructed on the correct use of the inhaler. An inhaler may also be left in the clinic in case of an emergency.
If a student has any medical condition which may need attention during the school day...i.e.: seizures,diabetes, heart disease, severe allergies...please call the clinic.
